Complete the official TFDA notification form.
Product names must be in Thai (English may be added) and must match the label.
Product category must follow TFDA’s classification system.
Formula ingredients should be listed in order, using INCI names or TFDA-recognized nomenclature, with percentages clearly indicated.
Letter of Authorization (LOA)
Must be within the valid period, specifying the scope and authority of authorization.
The authorizing party must be the brand owner or manufacturer, and the authorized party must be a legally registered Thai entity.
Product English Label (Inner & Outer Packaging)
Provide product packaging images (both inner and outer), showing the brand name and all readable label content as it appears on the market.
GMP Certificate (e.g., ISO 22716)
Provide valid certificates such as ISO 22716 or equivalent, showing the manufacturer’s name, certification scope, issuing authority, and validity period, demonstrating compliance with Good Manufacturing Practices.
Additional Supporting Documents
Depending on the product type, additional documents such as test reports, raw material information, or other technical files may be required.
